Where can I get copies of these documents for my state?

Each state has its own laws regarding advance directives and many states have their own forms. Federal law requires hospitals to provide information about advance directives to the people in their communities; contact the Patient Representative or Department of Social Services at a hospital near you and request copies. In most - but not all - states, you won't need to hire an attorney to prepare these documents.

You may also be able to pick up copies from your local Office on Aging, nearest chapter of the AARP, nearby senior centers or from assisted living and nursing facilities in your vicinity.
